For calendar years 2004 and 2005, SMF failed to submit in a timely manner the hazardous chemical inventory form to the Indiana SERC, Allen County LEPC and Fort Wayne Fire Department for the 4000 lbs of sulfuric acid it had on site. The company will pay the penalty in 8 equal installments.
