D/FD Operating Services LLC, on behalf of McKee Run Generating Station failed to timely submit its Form Rs for PAC's & Benzo (g, h, i) Perylene chemicals. They self disclosed these violations to EPA on June 13, 2002. After review of their disclosure letter & questionnaire, EPA determined that they met the self audit policy criteria and therefore the penalty was mitigrated.
