'VIOLATION: 1) Permit violation-WWTP not being properly operated and maintained; and 2) Pretreatment violations.
**RELIEF: 1) Within thirty days, the Permittee shall take corrective action to eliminate and prevent recurrence of the violations; 2) Within thirty days, submit a written report of specific actions taken to correct violations; and 3) If complete correction of violations is not possible within thirty days, submit a comprehensive plan to do so.
