'**VIOLATIONS: 1) Failing to prepare and deliver a CCR; 2) Failing to provide a copy of the CCR to the primary agency; and 3) Failing to provide certification of the distribution of the CCR. **RELIEF SOUGHT: 1) Prepare and deliver a CCR within 45 days; 2) Provide a copy of the CCR to the EPA and NMED/DWB; and 3) Provide certification of the distribution of the CCR to NMED/DWB and to the EPA within 15 days of delivery
