'VIOLATION: 1) Respondent did not make timely application for permit coverage; 2) Respondent did not develop a Pollution Prevention Plan; and 3) Respondent did not develop SWPPP.
**RELIEF: 1) Within thirty days, Respondent shall make application for permit and submit copy of application and confirmation of coverage; 2) Within thirty days, Respondent shall take corrective action to correct deficiencies and eliminate and prevent recurrence of violations, and submit report detailing specific actions taken; and 3) If complete correction of violations is not possible within thirty days, submit a comprehensive written report.
