The Overfield #2W injection well failed a mechanical integrity test (MIT) on July 6, 2000. The last successful demonstration of mechanical integrity of the injection well was on June 21, 1988. Respondent''s permit requires annual submittal of monthly injection monitoring reports. The last monitoring report submitted to EPA for the Overfield #2W injection well was for the 12 month period from July 1999 through June 2000. Respondent did not submit the injection fluid analyis reports yearly to EPA. No injection fluid analysis reports for the Overfield #2W injection well have ever been submitted to EPA.
