'**VIOLATIONS: 1) Failure to attach a statement disclosing either the presence of any known lead-based paint and/or lead-based paint hazards, or indicating no knowledge of the presence of lead-based paint and/or lead-based paint hazards; 2) Failure to include in the contract a statement by the purchaser affirming receipt of the information required and the lead hazard information pamphlet required; and 3) failure to attach a statement disclosing either a list of records or reports pertaining to lead-based paint and/or laed-based paint hazards in the housing or a statement indicating no such records or reports are available. **PROPOSED PENALTY: $1,760.00.
