The system failed to conduct total coliform monitoring during the quarterly compliance periods of April - June 1998, July 1998 - September 1998, October - December 1998, January - March 1999, April - June 1999, July- September 1999, October - December 1999, January - March 2000, April - June 2000, July - September 2000, October - December 2000, and January - March 2001; failed to conduct monitoring for nitrate in 1998, 1999, and 2000; failed to conduct nitrite monitoring by December 31, 1998; failed to report the above mentioned violations to the State within 48 hours.'
