'**VIOLATIONS: 1) Offering hazardous waste for transport without an EPA identification number; 2) Accumulating more than 6,000 kilograms of hazardous waste on-site without obtaining a permit; 3) Failing to meet the exemptions for storong hazardous waste for less than 90 days without having a permit or interim status; 4) Failing to submit a biennial report by March 1, 1999; 5) Accumulating hazardous waste in containers at or near any point of generation where eastes are initially generated that were opne and not marked with the words Hazardous Waste or other words that identify its content; and 6) Storing hazardous waste in a container that was open at times other than necessary to add or remove hazardous material, and by accumulating hazardous waste in containers at or near any point of generation where wastes are initially generated that were open and not marked with the words Hazardous Waste or with words that identified the contents of the container. **ASSESSED PENALTY: $20,850.00.
