'This is a Civil Judicial Report alleging that Chevron U.S.A., Inc., at its refinery in El Segundo, California, violated Risk Management Planning regulations stated in 40 C.F.R. Part 68, as promulgated under Section 112(r) of the Clean Air Act, and repeatedly failed to immediately notify the National Response Center, the State Emergency Planning Commission, and the Local Emergeny Planning Committees for the affected areas of releases of Hazardous and Extremely Hazardous Substances.'
