Dynacircuits, LLC, Franklin Park, Illinois failed to submit the completed Emnergency and Hazardous Chemical Inventory Form to the State Emergency Response Commission, the Local Emergency Planning Committe, and the local fire department with jurisdiction over the facility for calendar year 1999 by March 1, 2000.
