## Summary

Respondent operates injection wells in Osage ;County, Oklahoma as provided by mineral mining leases with the Osage Nation. Respondent is subject to regulations under the Safe Drinking Water Act, Underground Injection Control (UIC)program. Applicable UIC regulations require that Respondent maintain injection wells so that fluids containing contaminants cannot move though the well bore into underground sources of drinking water (USDWs). One of Respondent's injections wells has a static fluid level above the base of USDWs. This presents a threat that contaminated fluids could move through the well bore into USDWs. This order requires Respondent to conduct necessary work to assure that contaminated fluids would not move through the well bore into USDWs.
