Respondent operates injection wells in Osage County, Oklahoma as provided by mineral mining leases with the Osage Nation. Respondent is subject to regulations under the Safe Drinking Water Act, Underground Injection Control (UIC) Program. Applicable UIC regulations require that Respondent submit an annual report of well operations. The report must include average and maximum monthly injection pressure and volume of fluids injected each month. Respondent failed to submit the required reports for one injection well for the period October 2000 through September 2002. The AO requires Respondent to submit the required report.
