Respondent failed to maintain a record of inspections; in violation of Part I.B.3. Respondent failed to comply with the Final Limitations and Controls; in violation of Part I.C.1. Respondent failed to maintain provide operation and maintenance necessary to operate storm water controls at optimum efficiency; in violation of Part I.C.2. Respondent failed to properly operate and maintain all facilities and systems of treatment and control; in violation of Part II.C.
