Respondent failed to provide the operation and maintenance necessary to operate the storm water controls at optimum efficiency as evidenced by the discharge of sediments from the site and the lack of maintenance of silt fencing and at the sedimentation basins; in violation of Part I.C.2. Respondent failed to properly operate and maintain all facilities and systems of treatment and control to achieve compliance with the conditions of the Permit as evidenced by the discharge of sediments from the site and the lack of maintenance around the site; in violation of Part II.C.1 of the Permit
