Respondent failed to implement the approved Plan as required, failed to adequately install and/or maintain temporary or permanent measures to control and retain sediment and lack of adequate groundcover throughout the entire state; in violation of Part I.A.2 of Permit. Respondent failed to conduct inspections and maintain records of all inspections as required by the permit; in violation Parts I.B.1 and I.B.3 of the Permit. Respondent failed to maintain proper operation and maintenance of structural and storm water controls at the site as required by the Permit; in violation of Parts I.C.1, I.C.2, and II.C.1 of the Permit. Respondent failed to comply with Part I.!.2., Part I.B.1., Part I.B.2., Part I.C.1., Part I.C.2., and Part II.C.1. of the Permit.
