AO issued 11/20/2002. Respondent failed to properly operate and maintain all facilities and systes of treatment and control (and related appurtenances) which are installed or used by the permitee to achieve compliance in accordance with Part II.C.1 of the Permit. Respondent failed to comply with Final Limitations and Controls specified for storm water discharged once disturbance has begun on the site and until completion of construction or development and the establishment of a permanent gound cover, in accordance with Part I.C.1 of the Permit. Respondent failed to provide the operation and maintenance necessary to operate the storm water controls at optimum efficiency in accordance with Part I.C.2 of the Permit. Respondent failed to keep records of inspections and have them available for authorized representatives in accordance with Part I.B.3 and Part II.D.5.b of the Permit. Respondent failed to keep a rain gauge on site to document rainfall quantity, as required by Part I.B.1 of the Permit
