AO issued 11/20/2002. Respondent failed to maintain a copy of the approve Plan on site, as required by Part I.A.2 of the Permit. Respondent failed to inspect all erosion and sedimentation control faciliteis, as required by Part I.B.1 of the Permit. Respondent failed to take immediate action to control sediment discharge outside the disturbed limits, as required by Part I.B.2 of the Permit. Respondent failed to have records of inspections available for authorized representatives, as required by Part I.B.3. and Part II.D.5.b of the Permit.
