**RELIEF SOUGHT: 1) Within thirty days, Respondent shall make application for coverage under permit and shall submit to EPA a copy of permit application and confirmation of coverage; 2) Within thirty days, Respondent shall take corrective action and submit a written report detailing the specific actions taken to correct the violations; 3) If complete correction of violations is not possible within thirty days, submit a comprehensive plan to do so; and 4) Within forty-five days, arrange a show cause meeting with EPA
