VIOLATION: 1) Certified DMRs filed by Permittee show discharges of pollutants from the facility that exceed the permitted effluent limitations; and 2) Deficiencies found during TCEQ inspection.
**RELIEF: 1) Within thirty days, the Permittee shall take whatever corrective action is necessary to eliminate and prevent recurrurence of the violations and submit a written report detailing the specific actions taken; 2) If complete correction of the violations is not possible within thirty days, submit a comprehensive plan to do so; and 3) Within forty-five days, arrange a Show Cause meeting with EPA.
