VIOLATION: 1) Certified DMRs filed by the Permittee with EPA, show discharges of pollutants from the facility that exceed permitted effluent limitations; and 2) To incorporate Schedule from previously issued AO CWA-06-2002-0010.
**RELIEF SOUGHT: 1) The Permittee shall comply with the schedule listed in this Order for construction of a new wastewater treatment plant; and 2) The Permittee shall submit quarterly progress reports through completion of the project.
