AO issued 11/20/2002. Respondent failed to operate the storm water controls at optimum efficiency as required by Part I.C.2 of the Permit. Respondent failed to properly operate and maintain facilities and systems of treatment and control which are installed to acieve compliance with the Permit, as required by Part II.C.1 of the Permit. Respondent failed to comply with Parts I.A.5, I.B.1, I.C.2, II.C.1 of the Permit, and also for discharges not authorized by the NCDENR Permit.
