Respondent operates injection wells in Osage County, Oklahoma as provided by mineral mining leases with the Osage Nation. Respondent is subject to regulations under the Safe Drinking Water Act, Underground Injection Control (UIC) Program. Applicable UIC regulations require that Respondent submit an annual report of well operations. The report must include average and maximum monthly injection pressure and volume of fluids injected. Respondent failed to submit the required reports for two injection wells for the period April 2001 through March 2002 and for another well from October 2001 through September 2002. An administrative order was proposed on January 27, 2003, to require Respondent to submit the required reports.
