A compliance evaluation inspection was conducted at the Lake Air Mall construction site in Waco, TX, on 12/21/02. Middleman Construction Company, LLC, and HPRI - Waco Centers, LP, were two of the permitted operators. Deficiencies in SWPPP included an unauthorized signature on the HPRI certification of the SWPPP, the operators' inspector was not authorized nor were his qualifications notated in the SWPPP. An administrative order was drafted.
