VIOLATIONS: Maintaining injection well in a manner that could allow fluids to flow through the well bore into underground sources of drinking water
RELIEF: Either plug the well according to Environmental Protection Agency regulations, demonstrate that the well has mechanical integrity, or complete other appropriate corrective action within 30 days after the effective date of the Order.
