SUMMARY We have proposed five projects that together address the causes and consequences of healthcare efficiency, with a specific focus on gaps in the evidence, clinical dilemmas, and health policies relating to treating the frail elderly and those with Alzheimer’s Disease and related dementia. Core B is responsible for the comprehensive data management and analytic support on which the five projects rest. Our aims include first, to maintain a comprehensive and secure database of administrative data including Medicare and Medicaid enrollment files, claims records, and supplementary files developed under the Dartmouth Atlas and other projects. The key tasks under this aim entail maintaining access to an increasingly massive database and obtaining and preparing for analysis additional data as they become available. Second, to obtain and manage additional data files required for this program project grant and to develop project-specific analytic files. We will obtain data from a variety of sources specifically for this program project grant, including Medicare, Medicaid, the Minimum Data Set of nursing home care, the Health and Retirement Study, and the Vascular Quality Initiative. We would also coordinate and link data from Optum Labs and Blue Health Intelligence, and the Health Care Cost Institute data (through the National Bureau of Economic Research) on commercially insured under-65 and Medicare Advantage over-65 populations. As well, Core B would help to coordinate the use of several national Dartmouth surveys of health care providers. Third, we propose to create a combined data set that captures prices, quality, and utilization at the hospital referral region (HRR) level based on Medicare, Medicaid, and private insurance claims data. We then plan to develop alternative regional measures, including micro-based (ZIP code) spatial density maps to capture hot-spots in utilization and various indicators of care quality. Finally, in coordination with Core A, to make the research files developed under this program project grant available to the research and policy community.
