EE WAS INSTALLING 4' RESIN ROOF BOLTS INTO THE TOP. AS HE BEGAN TO INSERT ROOF BOLT INTO HOLE, THE BOLT BENT TRAPPING HIS FINGER BETWEEN BOLT WRENCH AND DRILL HEAD.
