THE EMPLOYEE WAS DRIVING FORK TRUCK AND RAN OFF THE SIDE OF THE ROAD. THE FORK TRUCK ROLLED OVER AND SMASHED EMPLOYEE, RESULTING IN A FATAL INJURY.
