EMPLOYEE WAS DRILLING HOLE FOR ROOF BOLT HE PUT BOLT WRENCH TO PUSH STEEL DAPER IN HOLE USED HIS HAND TO STEADY WRENCH. GLOVE GOT CAUGHT ON WRENCH. BREAKING HIS ARM.
