EMPLOYEE WAS BOLTING. HE WAS TIGHTENING THE BOLTWITH A WRENCH. THE WRENCH SLIPPED OFF THE RESIN BOLT AND STRUCK HIS UPPER LIP. REQUIRED SUTURES.
