Narrative

EE WAS STANDING ON A PLATFORM UNCHOKING A CLINKER FEEDER, WHEN THE PLATFORM COLLAPSED. TWO EMPLOYEES FELL APPROX 3 FT TO THE FLOOR BELOW. AN INVESTIGATION SHOWED A BOLT HOLDING THE PLATFORM HA D BROKEN.
