EMPLOYEE WAS WALKING A BELTLINE. AS HE WENT THROUGH A SET OF ARCHES, HE SLIPPED ON A ROCK, BUMPED HIS HEAD AND FELT A PULL IN HIS BACK. EMPLOYEE BECAME A LOST TIME INJURY.
