THE EMPLOYEE WAS CUTTING SCREEN TENSION BOLTS ON THE CRUSHER USING A TORCH AND A HAMMER. HE RAN HIS RIGHT HAND IN FRONT OF THE TORCH EXPOSING HIS WRIST TO THE FLAME.
