TRAVELING DOWN ACCESS ROAD TO QUARRY WHEN HE TOOK THE TRUCKOUT OF GEAR TO DOWNSHIFT AND COULD NOT GET IT BACK IN GEAR. ALSO, TRAVELING TOO FAST TO MAKE TURN CAUSING TRUCK TO ROLL.
