EMPLOYEE WAS CHIPPING CONCRETE WITH A 15 POUND JACK HAMMER. THE HAMMER BOUNCE UP AND STRUCK HIMIN THE MOUTH. INJURY CONSISTED OF 3 LOOSE TEETH THAT HAD TO BE PULLED.
