Narrative

SUPERVISOR WENT TO THE #4 SHAFT HOIST ROOM ON 3100 LEVEL IN AN ATTEMPT TO RESET THE HOIST AND FOUND THE HOIST ROMM FULL OF SMOKE. THERE WERE NO SIGNS OF FIRE.
