Narrative

EMPLOYEE & CO-WORKER HAD USED A COME ALONG TO UNLOAD ELECTRICAL CONTROL BOX FROM BACK OF VEHICLE. CONTROL BOX WAS SUSPENDED IN AIR AND BEING LOWERED TO MINE FLOOR WHEN CLEVIS BROKE AND CHAIN HOOK HIT EMPLOYEE ON FOOT.
