Narrative

WHILE ROOF BOLTING A PIECE OF SHALE FELL AND STRUCK HIM ABOVE UPPER LIP RIGHT SIDE. THIS CAUSED A LACERATION THAT REQUIRED STITCHES. HE WAS TREATED AND RELEASED TO RETURN TO WORK.
