EMPLOYEE WAS USING A GUZZLER TRUCK TO CLEAN UP A SPILL HIS ARM STUCK IN THE END OF THE HOSE. EMPLOYEE DID NOT HAVE THE VACUUM BREAK IN PLACE. EMPLOYEE WAS PLACED ON RESTRICTED DUTY.
