Narrative

WHILE RUNNING CENTERS IN THE FACE HE STRUCK A ROOF BOLT WITH HIS HARD HAT. HE COMPLAINED ABOUT NECK AND SHOULDER PAIN. HE CONTINUED TO WORK BUT WAS ALSO SEEING A DR. ON 10-13-00 HE RECIEVED AN MRI. ON 10-18-00. HE WAS DIAGNOISED WITH A POSSIBLE HERNIA OF THE 4TH CERVICAL VERTEBRAE HE HAS NOT MISSED WORK MEDICAL ONLY WITH RESTRICTIONS.
