A STEEL WEDGE WAS BEING USED TO REMOVE THE VERTICAL SWING SHAFT ON A SHOVEL. WHILE HAMMERING THEWEDGE SIDEWAYS TO REMOVE IT, IT KICKED OUT & STRUCK EMPLOYEE'S LEFT INDEX FINGER, FRACTURING IT.
