Narrative

MAN WAS INSTALLING ROOF BOLT IN HOLE, UNBENDING BOLT, PLACED BOLT HEAD IN CHUCK AS HE WAS RAISING BELT WITH BOLT HEAD. HIS GLOVE WAS CAUGHT BY ROOF BOLT PLATE AND WAS TWISTED AROUND ROOF BOLT.
