Narrative

EMPLOYEE WAS HELPING UNLOAD BELT FROM A FLAT CARBY USING PLIERS TO HOLD ONTO THE BELT IN ORDER TO GRIP THE BELT. THE PLIERS SLIPPED WHICH LET HIM FALL CAUSING THE INJURY.
