AT THE BOTTOM OF THE 20' INTAKE SHAFT. A ROOF FALL OCCURRED ABOVE THE ROOF BOLTS BEGINNING AT THE BROW LINE OF THE NORTH ENTRY. THE FALL WAS APPROX. 14' WIDE, 15' INTO THE ENTRY, AND 8' ABOVE ORIGINAL TOP.
