ON 4/14/01 THE MINER WAS IN A PRODUCTION TRUCK. A CHUNK FELL OUT OF THE BUCKET OF THE SHOVEL ANDSTRUCK THE BOX OF THE TRUCK JARRING HIM. BECAME AWARE OF RW ON 4/26/01.
