EMPLOYEE WAS ON THE LEFT SIDE OF THE CONTINUOUS MINER WHEN A PIECE OF LOOSE RIB COAL FELL CAUSING HIS LEFT LEG TO BE CAUGHT BETWEEN THE LOOSE COAL AND THE CONTINUOUS MINER MACHINE.
