PUTTING UP 6FT. BOLT. EMPLOYEE WAS BENDING BOLT BACK INTO SHAPE WHEN PLATE SLID DOWN BOLT. EMPLOYEE STRUCK THE PLATE WITH THE BACK OF HIS RIGHT ARM CAUSING A CUT THAT REQUIRED 8 STICHES TO CLO SE.
