EE WAS INSTALLING ROOF BOLTS IN AN INTAKE AIR COURSE, USING A SINGLE BOOM ROOF BOLTER. THE ROOF BOLTING MACHINE WASS SITTING ON AN UNEVEN MINE FLOOR, & AS HE WAS INSTALLING A ROOF BOLT, HE WAS HAVING DIFFICULTY INSERTING THE ROOF BOLT HEAD INTO THE BOLT WRENCH. HE USED THE PALM OF HIS HAND TO FORCE THE BOLT HEAD & WRENCH TOGETHER, PALM WAS PINCHED BETWEEN WRENCH & BOLT HEAD. 8 SUTU
