EMPLOYEE STATED HE WAS WALKING UP THE #3 ENTRY TO OBSERVE TAKING THE PUSH OUT IN #41 PILLAR BLOCK WHEN PIECE OF RIB MATERIAL ROLLED OUT ONTO THE MINE FLOOR BREAKING UP INTO PIECES. ONE OF THE PIECES ROLLED ONTO HIS FOOT CAUSING HIM TO FALL.SEVERE DEGENERATIVE DISC DISEASE OF THE LUMBAR SPINE. SYNOVITIS AND ARTHRITIS OF THE RIGHT FOOT AND LEFT KNEE.
