OCCURRED AT 9:15PM ON 10/18/01. FALL LOCATION WAS AT SURVEY STATION 23+10 #5 5LMS SECTION 006 THE FALL IN A 4 WAY INTERSECTION WAS 20'LONG, 30' WIDE, 10' HIGH. AREA WAS MINED 8/2001 AND HAD BEEN SUPPORTED WITH 8' FULL RESIN PINS & WAS TRUSS BOLTED NO INJURY RESULTED BUT IT DAMAGE INTAKE STOPPING & SUPPLY CAR OF STOPPING MAT.
